APPOINTMENTS

Office visits are by appointment only. If you have an acute condition, please call the office and we will schedule you as soon as possible.

PATIENT PORTAL

Our clinic uses a free, secure, and HIPAA-compliant health records management and message portal system called 'Patient Ally'.  It is available to newly established patients upon providing information to the receptionist and scheduling an initial appointment.  New patient forms will be routed to your Patient Ally account for completion and can be found in the 'Documents' section of Patient Ally.  Please complete and submit the forms 2 business days prior to your appointment to allow for office staff to contact you with any questions.  This way, the time during your visit can be used most effectively to address your health concerns.

If you have not already set up a Patient Ally account under a different provider's clinic, our office staff can initiate the account set-up link.  If you have forgotten your username or password or are having difficulty, click https://www.patientally.com/info/faq   or call 1-888-PHR-4ALL (747-4255) to speak with Patient Ally technical support.

To access Patient Ally's Patient Portal, you must first receive an email from your provider's office to initiate the set-up process.  You must then verify some patient questions and validate your identity using the cell phone number and email on file with our clinic.   Once you have created a username and password, you have completed the enrollment process and now may begin using Patient Ally.

Notable features of Patient Ally include:

  • Secure completion of clinical documents for distance/virtual/telephone patients

  • Ability to receive or send documents including laboratory requisitions and reports from other providers

  • Due to legal restrictions about the use of children's health information online, the parents of pediatric patients will be required to set-up their own accounts and then use them for some of the services above

  • Request doctor's appointments

  • Message the clinic

  • And more!!

  • Though there is an option to receive special offers, coupons, and patient vouchers, our clinic recommends declining this offering

NEW PATIENT INTAKE FORMS

Please open, download and complete the PDF form prior to your first visit, but

only if you have trouble establishing an account and completing these through

the Patient Portal.

Cancellations

If you need to reschedule an appointment, please give the office at least 24 hours notice, applicable to business days. This courtesy will allow another patient to be able to see the physician. Missed visits without 24 hour notice, applicable to business days, will result in a cancellation fee. Phone call reminders are a courtesy of the office, but it remains the responsibility of the patient to ensure they keep their appointment or cancel with sufficient notice.

Credit Card Policy

Credit cards are required to be kept on file when scheduling. Credit card authorizations are for the financial settlement of bills for services, insurance copayments, telephone consultations, missed appointments / cancellation of appointments with less than 24-hour notice, and purchases from Northeast Natural Medicine, LLC.

Dispensary

For the convenience of our patients, we carry some medical grade supplements. To maintain the quality of the product we carry, we cannot accept returns once they have been opened, used or after being off our temperature controlled shelves for more than 30 days. Refrigerated items, custom tinctures or special orders cannot be returned. A full credit or refund will be issued if the supplement is returned unopened, within 30 days. Many items can be purchased at a discount on our online dispensary. Supplements purchased through the online store cannot be returned to Dr. Carney’s office; please make your purchases wisely.
